Patty’s Place Concierge Youth Mental Health Care is a resource hub that provides support for youth ages 10 – 26 and their families. Our mental health professionals conduct a thorough Initial Intake Meeting during which they assess the individual and families needs. From there we offer Personal Planning Meetings with a team member that we call PALS, Professional Advocates Linking Support. We develop support plans, offer guidance, and provide monitoring of an individuals progress. If additional long term support is recommended or specialty care is required, we ensure that families get connected to the appropriate community providers. By having reciprocal referrals between Patty’s Place and community providers it will help to decrease potential wait times that may be encountered while seeking the right support. Our concierge model is aimed to help a person at the time of need and to determine their specific individual needs on their journey to wellness.
